C. Classes

1. Course notes will be provided per student per workshop. Apart from the course notes, students will need to bring their own materials such as pens, pencils, highlighters, etc.
2. No food and drinks are allowed inside the classroom.
3. No parents/guardians are allowed inside the classroom
4. Parents/guardians are advised to keep their child/children at home if he/she/they have colds, fever or any infectious disease. The Centre reserves the right to remove from the class any student who is sick (e.g. incessant coughing, fever above 38.5 degrees Celsius, sore eyes, etc.) for the safety of the rest of the class.
5. Parents/guardians are advised to bring their children to the toilet before the lesson starts.
6. Students are required to take care of their personal belongings. The Centre will not be responsible for any loss or damage to properties.
7. Students are expected to observe proper conduct during the lessons. The teacher/center reserves the right to remove any student showing extremely disruptive behavior from the class.
8. The Centre reserves the right to take any disciplinary action in the event of misconduct, usage of abusive language, theft, or other infringement of other people’s safety.
9. Students are required to take care of their personal belongings. The Centre will not be responsible for any loss or damage to properties.
10. Students or parents are liable to bear the costs of repair or replacement in the event of damages occurred due to student misconduct or misbehavior.
11. The center reserves the right to reduce the duration of the class or dissolve the session/s when deemed necessary.

I AGREE TO:

• I have read and agree to abide by the terms and conditions listed above. I do hereby declare that I shall not hold The Music Works Centre responsible for any mishap or loss that I or my child may sustain while participating in any of the activities organized by them.
• I fully understand and agree that the personal information which I have provided may be used for purposes related to the music courses of The Music Works Centre.
• I hereby give consent to The Music Works Centre, in respect of the photographs, interviews, or audio or video tape recordings and their transcripts, that it has taken of me or in which I may be included with others, to use, reuse, publish, and republish, and otherwise reproduce, modify and display the same, in whole or in part, individually or with other photographs, in any and all media now or hereafter known, for illustration, promotion, art, advertising, and trade, or any other purpose whatsoever; and to use my name if it so chooses. I hereby release and discharge The Music Works Centre from all claims and demands arising out of, or in connection to, the use of these materials.
